Meticulous Planning

before the first incision.
Planning considers what must be protected, what tissue is available and what the defect will ask of the repair over years, not weeks.
The simplest reconstruction that will do the job is usually the right one. Complexity is chosen when it is needed, not for its own sake.
Imaging, measurement and rehearsal happen ahead of the operation, so decisions in theatre are confirmations rather than improvisations.
Recovery, further stages and the eventual scar belong in the plan from the beginning.

Tissue grafts
Skin, fat, nerve, tendon, cartilage or bone taken from elsewhere on the body to replace what is missing.

Local & regional flaps
Tissue moved from beside the defect on its own blood supply, so the repair matches the site in colour, thickness and texture.

Free flaps & microsurgery
Tissue transferred from a distant site and its vessels rejoined under the microscope, for defects too large or complex for local repair.
